Terms and Conditions
Below are the terms and conditions on which I, Jane Wijeratna of Physio Difference, provide physiotherapy services.
Initial Assessment:
Your first appointment offers time for an assessment of your condition, a discussion with you on its management now and in the future and a structured way forward.
Please note that after the Initial Assessment has been carried out, I may decide that I cannot provide any Services if, for example, treatment for the condition may not be suitable or appropriate.
If treatment is deemed to be suitable then your first session of physiotherapy will be done either at the same appointment if appropriate and time allows, or at the subsequent follow-up treatment session.
Sessions:
If further appointments are agreed with you to be necessary these will be made with you by telephone, in person or by e-mail.
Exercise Plan:
I may set you a functional exercise programme to be completed between sessions. You are not obliged to complete these but if you do not then your progress in achieving your full potential may be slowed down.
Fees:
The fees for each session are as confirmed by me to you. You are personally liable to pay all of the fees and any surcharges incurred, such as any cancellation fees.
Payment:
I accept payment in cash, cheque, bank transfer, debit or credit card.
Late cancellation or missed appointments:
If you are late starting at a session, the session will begin on your arrival and continue until the time when it is scheduled to end.
If you have booked a session and you cancel with less than 48 hours’ notice or do not turn up for the session, then you are liable to pay the cost of that session.
On occasion, I may have to cancel a scheduled session. This may occur, for example, if I am sick. If I have to cancel a scheduled session, I will book you another appointment as soon as reasonably possible. No charge will be made to you for the session cancelled by me.
Indemnity:
You agree that you will indemnify Physio Difference against any and all loss or damage suffered, including any legal fees or costs, as a result of any breach of this agreement by you.
Early termination:
In exceptional circumstances, such as illness or other commitments, inappropriate behaviour by you, refusal to be treated in a reasonable way, actual or potential conflict of interest, or other reasons, Physio Difference may decide to terminate the Services early and or refuse or be unable to provide further sessions to you. In such circumstances, Physio Difference shall give you reasonable notice of termination where practicable and will refund to you any advance payment made for sessions not yet provided.
Privacy Policy:
Jane Wijeratna is a Chartered Physiotherapist who works as a sole trader under the name Physio Difference. This privacy policy sets out how Physio Difference stores and manages your personal data.
Any personal information, including your medical history is safe and confidential and the information is stored on a secure server.
Physio Difference never shares your personal data for marketing or mailing purposes and will only share your medical information with other medical professions with your knowledge and consent.
Your personal details:
Physio Difference treats all your personal data with confidentiality and during assessment and treatment, personal data such as your name, address, date of birth as well as health data, such as your past medical history and medication list will be recorded.
Physio Difference will also store contact details (name, phone, email) of your next of kin, as an emergency contact and if appropriate for communication regarding treatment outcomes and progress.
All personal data is either password protected or stored on a secure web based practise management software.
Data retention:
Physio Difference will keep your personal data including your medical physiotherapy records for 8 years, as recommended by the Chartered Society of Physiotherapy. After the completion of treatment your documentation will be archived and then deleted after the 8 year period.
Data access:
Patients have the right to request a copy of their personal data.
If any of your personal data is incorrect or has changed please inform Jane Wijeratna and it will be promptly amended.
Data breach:
If there is a data breach Physio Difference will notify you and the ICO (Information commissioner’s office) within 72 hours.
Marketing communications:
Physio Difference does not send any marketing communications, and does not share your data with any other company.
Links to other websites:
The Physio Difference website may contain links to enable you to visit other websites of interest easily. However, once you have used these links to leave the Physio Difference site, you should note that I do not have any control over that other website. Therefore, Physio Difference cannot be responsible for the protection and privacy of any information which you provide whilst visiting such sites and such sites are not governed by this privacy statement. You should exercise caution and look at the privacy statement applicable to the website in question.
